To complete your application online and submit it, please follow the steps below:

1. Double-click on the form above.
   
2. A new Microsoft Word document will open
   
3. Please type the required information into the application's highlighted fields.
   
4. When completed, click on the “printer symbol” or "save file symbol" to process submission by mail or email.
   
5. If printing, please mail the completed application and payment to the P.O. Box listed at the top of the application form.
   
6. If saving, please send the completed application file to Ms. Gloria McCoy (Membership Chair) at email address; gmccoy78@hotmail.com and mail payment to PO Box. Membership becomes effective when both are received.
